After 30 years of service to the community, the office of Dr. Jeffrey Lee, oral and maxillofacial surgery, has closed its doors permanently as of April 1, 2024.

For any past patients, we want to assure you that your medical records will be securely stored and accessible to you upon request. If you would like to obtain a copy of your medical records, please send a written request to:

Or email frontoffice@drleeoms.com. Records will be sent to the mailing address provided within 30 days. Kindly note that a request to copy will incur a fee of $30 per chart which include postage. Our record retention is 7 years. Any records before 2016 have been destroyed and purged. A notice of “no records” is available upon request for any patient seen prior 2016.

We understand that continuity of care is essential, and we are committed to assisting you during this transition period. If you require any further oral surgery assistance, we are referring all our patients to Dr. Lee’s trusted colleague, Dr. Hooman Adamous. We are confident that Dr. Adamous will provide you with the same level of care and attention. The following is his information:
